Assessment tool
The core of the assessment tool is to examine and analyse 9 dimensions regarding the person in question:

The assessment tool is dialogue based. The participating authorities bring each their information about the person to the table and discuss each dimension one by one. Part of the tool is comprised of guidelines, listing points of attention regarding the nine dimensions, all derived from evidence based research.
In its entirety the assessment model is comprised by four tools which guide the practioners in the infohouses through three steps:
- Reception and description of the concern.
- Analysis of the situation of the person.
- Comprehensive assessment and recommendation
